Personalized Memory Care Programs at Cedar Creek

Comprehensive Dementia and Alzheimer’s Support

Each Cedar Creek home offers activities and therapies to promote socialization and cognitive stimulation. When a new resident arrives, we compile a short biography that highlights their life, focusing on their accomplishments, talents, hobbies and things that bring them joy. This is then added to our “Family Album,” which serves as the cornerstone of our activities program and gives staff members an idea of who the residents were before they started experiencing memory loss. Each home’s activity program is unique, based on residents’ needs and preferences, but these structured and informal interactions allow us to reach out to the whole person and provide comprehensive support.

Individualized Care Plans

Our individualized care plans are as diverse as our residents. Before we can craft the perfect plan, we assess a resident’s cognitive status, their likes and dislikes and their personal, emotional and medical care needs. This allows us to offer them comprehensive care while maintaining personal autonomy.

Features and Amenities Designed for Memory Care

Comfortable Environment

Cedar Creek’s intimate memory care communities were purposefully designed to be small and homelike, with spacious accommodations, comfortable living areas and secure outdoor spaces. A smaller environment with low resident numbers helps minimize confusion and promote feelings of comfort and security.

Engaging Activities and Therapies

Our residents regularly participate in fun and engaging memory care activities and therapies organized by our full-time activity therapist, such as reminiscence therapy, movement activities and sing-alongs. Residents benefit greatly from this program as participation promotes social engagement, sensory engagement and cognitive stimulation. It also helps prevent boredom and forge a sense of belonging.

Cedar Creek’s Commitment to Families

Family Involvement in Care

It’s not uncommon for seniors with Alzheimer’s or dementia to feel isolated or disconnected from others. This is why we’re so committed to keeping families involved in their loved ones’ care. We work with them to create a personalized care plan and then provide regular updates to keep them in the loop. Family members often underestimate the value of their participation in their loved one’s memory care, so we encourage them to visit often and connect with staff when they have concerns.

Community Feel and Support

Cedar Creek’s memory care homes are communities that embrace everyone who works and lives there. Compassionate and nurturing staff members provide around-the-clock support, while residents are encouraged to socialize and form meaningful relationships. What Families Say

Frequently Asked Questions

Daily rates are all-inclusive and do not change as care needs increase. Cedar Creek does not charge extra for personal care, medication administration, laundry services, or housekeeping. Residents do pay for their own medication, personal care items, and supplies.

We have doctors, nurses, therapists, dentists, podiatrists, and other health care professionals who visit residents at our Alzheimer’s care homes. Residents are welcome to continue to see their own doctor, but we provide the option to utilize our doctors for convenience and for more monitored medical oversight.

Cedar Creek residents may receive a doctor’s order for physical therapy, occupational therapy, or speech therapy. Therapists can come to any of our Alzheimer’s care facilities to work with our residents.

We will work with you and their doctor(s) to obtain the necessary information for the assessment. Once the decision has been made to move to Cedar Creek, we will assist you in locating a doctor who can be their new health care practitioner if needed.

We have a unique and individualized activity program, structured to meet the needs of residents with Alzheimer’s disease and memory loss. Our amazing full-time activity therapist learns about each resident’s background and finds opportunities to bring them joy. You may find our activity therapist outside working with residents in one of our retirement community’s vegetable gardens, taking residents on a journey with her iPad to explore their hometown, or leading a reminiscence group. If our activity therapist meets a resident with an interest in quilting, they start a quilting group. We also have therapists who provide cognitive stimulation and entertainers who provide music therapy for our residents. In addition, our caregivers are trained to engage residents in meaningful activities throughout the day, such as leading daily exercise or sing-a-longs, working on puzzles, and art projects. Our activity program varies based on the home and the interests of the residents, which is why we can truly say it is unique.

In most cases, residents stay with us for long-term care throughout their lives. Additionally, residents whose health status qualifies for hospice care can be assisted at our residential memory care homes by nursing support from local hospice organizations. In rare cases, when a resident’s care needs exceed the licensed capabilities of Cedar Creek, we work closely with family members to find the best alternative and promote a smooth transition to the new care facility.
