Person-Centered Dementia Care

If you’ve ever watched a loved one go through dementia or Alzheimer’s disease, you know it isn’t easy. Many people with dementia experience physical and cognitive changes that can cause them to lose their sense of self.

At Cedar Creek Memory Care Homes, we focus on person-centered dementia care that highlights the individual needs of each of our residents. Let’s take a closer look at what our homes have to offer.

Dementia Care Near Me

Our dementia care facilities in Maryland are near the following locations:

It can be difficult to provide personalized care in large, institutionalized settings, which is why we maintain small environments — in fact, many residents remark that our homes resemble houses they once lived in. Not only does this help ensure that every resident’s needs are met, but it also fosters a sense of community.

All of our homes are located in Montgomery County, a convenient, picturesque region of Maryland. Residents enjoy the peaceful feel of having their own homes while still receiving the support and care they need.

Quality Dementia Care

At Cedar Creek Memory Care Homes, we recognize that each individual has their own experience with Alzheimer’s disease and related dementia conditions. Quality care is paramount when it comes to finding suitable long-term care for Alzheimer’s patients. By understanding dementia behaviors, we’re able to provide the unique medical support every resident requires. This traditional medical model includes the following:

  • Assistance with daily living activities (like bathing, dressing and grooming)
  • Personal care
  • Incontinence care
  • Medication management
  • Nutrition monitoring

Our team is capable of serving all stages of memory care and adapting to the ever-changing needs of people with dementia. Along with our on-site staff, we offer access to external providers (including physicians, dentists, therapists and podiatrists). The ultimate goal is to make sure every resident remains happy and healthy in our home.

Holistic Memory Care Activities

When picking a memory care home for a loved one or family member, many people focus on medical care. While high-quality medical assistance is essential, it’s also important to find a home that offers entertainment and activities, providing for the resident’s total well-being and not simply meeting their medical needs. According to the National Institute on Aging, participating in meaningful activities can extend lifespans and boost physical and mental health. At Cedar Creek, we offer a variety of holistic memory care activities, such as:

  • Fitness classes
  • Reminiscence activities
  • Musical sing-alongs
  • Movie nights
  • Daily exercise programs

Rather than having a set schedule of activities, we have individualized interventions with an activities therapist who works closely with residents to develop programs that meet their interests. We also encourage residents to assist around the house, which helps build a sense of purpose. Ultimately, the goal behind our individualized activities is to help residents to find joy in the moment.

Dedicated Caregiver Staff

We wouldn’t be able to provide our immersive, individualized care if it weren’t for the dedication of our staff. While the healthcare industry has a high turnover rate, many of our caregivers have been with us for several years. This consistency has given our staff the opportunity to personally connect with residents and get to know their backgrounds, interests and family members.

In addition to longevity, we offer small staff-to-resident ratios, which is essential to creating our personalized care plans. Of course, our staff does more than provide medical care — they also provide residents with the love and support they need in times of need. Thanks to our small, tight-knit community, it isn’t long before our residents start to feel like family here.

Resident Biographies

According to the Alzheimer’s Association, it’s common for people with dementia to feel isolated as their dementia progresses. That’s why we make the extra effort to connect with residents beyond their disease and to forge family-like bonds. One way we accomplish this is by including a “Lifestyle Assessment” section on our pre-admission paperwork. This lets us gain insight into who the person was before they received their diagnosis. We then take this information and develop it into comprehensive “Resident Biographies” that cover the following topics:

  • Hometowns
  • Family member details
  • Occupation and interests
  • Accomplishments and talents

To make sure our team actually retains this information, staff members are quizzed on the biographies as part of their training. Not only does this help us get to know our residents as individuals, it also enables us to take better care of them. Being able to recall personal details about a resident’s family or interests helps reduce fears and anxiety (especially for those who struggle with communication).

Comfortable Amenities

As part of our person-centered care approach, we provide additional amenities designed to keep residents healthy and comfortable. Every resident enjoys the following:

  • Family-style dining
  • Housekeeping and laundry
  • Supportive technology

Having access to nutritious foods can have a huge impact on overall well-being, which is why we give each resident three healthy, homemade meals a day (as well as snacks). Because we maintain a small staff, we’re able to cater to different dietary preferences, as well as requirements and allergies.

Along with our delicious dining, we offer both housekeeping and laundry services. By taking care of maintenance chores, residents have more free time to socialize, build communication skills and engage in their interests. Finally, we offer supportive technology that helps residents maintain connections. For example, all of our homes are equipped with group iPads that are used to facilitate virtual connections.
